GENERAL INFORMATION

This information is necessary before a death occurs to make the process as smooth as possible. You will want to have the following information for everyone, in your family or care, in a safe place and readily available:

Name
Social Security Number
Date and Place of Birth
Armed Forces Service?
Usual Occupation (if retired, what they did before)
and in What Kind of Business
Marital Status
Spouse's Name
(even if deceased; if wife, have maiden name)
Residence
Highest Level of Education
Mother and Father's Names

Completing the death certificate will be much easier having this information on hand.

You will also want to know how many certified death certificates will be required to place the deceased's final affairs in order. You will need certified death certificates for life insurance claims, title transfers and Social Security claims. The fee in the State of Colorado is $15 for the first death certificate and an additional $6 for each certificate thereafter.

If your loved one is terminally ill you will want to notify your Hospice or care facility that THE DOVE SOCIETY will be handling the final arrangements. Have them call (303) 460-1120 anytime during the day or week. We are here to promptly take care of your cremation and/or funeral needs.
